Part I: Report of Events

Description of Event:

On August 14th, a contract employee was working inside Unit 1 scroll case using an electric metabo grinder from a 4’ step ladder with a 3” cup rock stone to grind down welds overhead around filler plates on the top of the wicket gates.  The employee was wearing prescription safety glasses with side shields under a universal style face shield that was attached to his MSA V-guard hardhat.  In the process of this task the employee stated that he felt dust/debris falling across his face multiple times during the execution of the task, but never experienced any discomfort on any part of his face, eyes, etc.

After completing the grinding activity, the employee began performing other duties unrelated to the incident.  Later in the afternoon, the employee began housekeeping activities to tidy up the work area.  Once he was finished, he washed his face to remove any dust/dirt that had collected during the shift and then exited the jobsite.

Immediately upon arriving to his quarters he took a shower and during that he experienced what he felt like was debris move across his left eye.  He immediately started attempting to flush his eye in the shower, but was unsuccessful at relieving the irritation.  The employee reportedly continued to rub his eye to attempt to relieve the irritation, but was unsuccessful throughout the evening.  The employee eventually sought medical attention and the doctor was able to remove the debris from his eye.

Lessons Learned

  • During the pre-job brief the job supervisor did not specify that spoggles/googles were required under the face shield for grinding activities.
  • The face shield was attached to the hardhat but was not compatible with the hard hat being used which left a gap between the hard hat and the face shield frame.
  • No HEPA vacuum system was used to remove dust and debris from the employees clothing or PPE prior to removal.
